As a social work candidate in Arizona, you must complete the following requirements to apply for the Licensed Baccalaureate Social Worker (LBSW) after graduating from the University of Phoenix Bachelor of Science in Social Work program.

Clearances:

Candidates must submit a full set of fingerprints and proof of online payment for a criminal history background check through the Arizona Department of Public Safety unless candidates can provide verification that they hold a current, valid DPS fingerprint clearance card.

Exam:

Bachelor's candidates must pass the Association of Social Work Boards (ASWB) baccalaureate exam. The ASWB Bachelor’s Exam can be completed with the authorization from the Arizona board. Authorization is given following completion of all other application requirements. If an applicant has completed the Bachelor’s exam in another jurisdiction, they should request that verification of a passing score be sent to this Board from ASWB. Candidates must receive a passing score on the approved ASWB exam within 12 months after receiving written examination authorization from the Board. An applicant shall not take an approved licensure examination more than three times during the 12-month testing period.

As a social work candidate in Arkansas, you must complete the following requirements to apply for the Licensed Social Worker (LSW) after graduating from the University of Phoenix Bachelor of Science in Social Work program.

Clearances:

All applicants for social work licensure must apply for a criminal background check and the results must be received by the Arkansas Social Work Licensing Board prior to being issued a provisional license, license through endorsement, or approval to take the exam. A current and valid application for licensure must be on file prior to criminal background checks being processed. Instructions will be sent for the background process once the application and fee have been received by the Board.

Exam:

After an application is approved, the applicant will be mailed a letter of eligibility with a link to the Association of Social Work Boards (ASWB) website for the candidate handbook and instructions for registering for the exam. The handbook contains general information about the examination, the registration process, and an order form for a study guide.

As a social work candidate in Delaware, you must complete the following requirements to apply for the Baccalaureate Social Worker (BSW) after graduating from the University of Phoenix Bachelor of Science in Social Work program.

Clearances:

Candidates must submit a State of Delaware and Federal Bureau of Investigation criminal background check (CBC). For instructions to complete your fingerprinting for both the Federal and State criminal background checks, go to IdentoGo. Candidates must also submit a completed and signed Child Protection Registry Consent Form.

Exam:

Bachelor's candidates must pass the Association of Social Work Boards (ASWB) baccalaureate exam. The ASWB Bachelor’s Exam can be completed with the authorization from the Delaware Board. Authorization is given following completion of all other application requirements. If an applicant has completed the Bachelor’s exam in another jurisdiction, they should request that verification of a passing score be sent to this Board from ASWB. Candidates must receive a passing score on the approved ASWB exam within two years after receiving written examination authorization from the Board. An applicant who does not pass the exam within two years must re-apply.

Clearances:

A Criminal Background Check is required for all health care license, certification, and registration applications. The applicant for a license is required to pay the fee established by the Department of Health for the criminal background check. The results of the criminal background check must be sent directly to the Board of Social Work.

Exam:

Bachelor's candidates must pass the Association of Social Work Boards (ASWB) baccalaureate exam. The ASWB Bachelor’s Exam can be completed with the authorization from the District of Columbia board. Authorization is given following board approval of the application.

• One recent and Identical Passport Type Photo (2x2 size) of the Applicant’s Face. The photo must be original and cannot be a computer-generated copy, or paper copy.
• One photocopy of a current government issued photo ID. This can be a driver’s license or passport.
• Three character reference forms. The names and addresses of three responsible persons (2 must be professional social workers and the 3rd can be family member or friend) who have known you for at least one year and can attest to your character. References must email these directly to the Board dcbosw@dc.gov. Applicants cannot email this form. Form must be emailed after an application has been submitted.
• Work Experience Form that lists all social work-related experience on form in chronological order. This form must be completed by all applicants and uploaded with application documents online.
• Vaccination Attestation via the Attestation Portal.

Clearances:

Applicants must disclose any complaints, charges of unlicensed activity, or pending disciplinary actions for any profession, occupation, or license, both motor vehicle and those other than motor vehicle on their application along with a signed, detailed statement and related court/agency documentation. If the application indicates criminal conviction, they must also submit a current criminal history record check from the Hawaii Criminal Justice Data Center (HCJDC) dated within six months. If the criminal conviction occurred in a state or states other than Hawaii, a current criminal history record check will be required from each state and Hawaii.

Bachelor's candidates must pass the Association of Social Work Boards (ASWB) baccalaureate exam. The ASWB Bachelor’s Exam can be completed with the authorization from the Department. Authorization is given following completion of all other application requirements. As a social work candidate in Illinois, you must complete the following requirements to apply for the Licensed Social Worker (LSW) after graduating from the University of Phoenix Bachelor of Science in Social Work program.

Clearances:

During the application process, candidates will complete the Personal History Information section that asks about previous convictions, health, previous denial of a professional license or permit, discharge other than honorably from the armed services or from a city, county, state, or federal position.

Post-Graduation Supervision Requirements:

Persons applying as a Licensed Social Worker who have a baccalaureate degree in social work shall complete 3 years of supervised professional experience subsequent to obtaining the baccalaureate degree. The experience shall be:

• Obtained under the direct supervision of a Licensed Social Worker under the Clinical Social Work and Social Work Practice Act (formerly Social Workers Registration Act), diplomate in clinical social work, designated member of ACSW or other appropriate supervisor as approved by the Board.
• Satisfactory as evaluated by the supervisor. The supervisor shall have met with the individual an average of at least 4 hours each month.

• NON-EXAMINATION – LSW ONLY. An LSW seeking licensure under Illinois Public Act 102-0326 is not required to complete an examination. This licensure method does not apply to LCSW licensure.

Clearances:

Candidates must complete a criminal background check as part of the application process. Candidates will be emailed when to start the background check process when the Indiana Professional License Agency receives a complete application packet.

Exams:

Bachelor's candidates must pass the Association of Social Work Boards (ASWB) bachelor-level exam. The ASWB Bachelor’s Exam can be completed with the authorization from the Indiana Professional Licensing Agency (scroll down to the Exams & Testing section and expand the Social Worker Examination section). Once approval from the agency is received, the agency will email you explaining the registration and scheduling process for the exam. An applicant who has been approved by the Board to take the examination must take the examination within one (1) calendar year from the date of the initial Board approval. If the applicant has not taken the examination within one (1) calendar year from the date of the initial Board approval, the approval will be invalid and the applicant must submit a new application and all required documentation must be resubmitted. No extensions will be granted.

As a social work candidate in Massachusetts, you must complete the following requirements to apply for the Licensed Social Worker (LSW) after graduating from the University of Phoenix Bachelor of Science in Social Work program.

Clearances:

In accordance with Board Policies and Guidelines (Social Workers) Policy on Criminal Convictions or Pending Criminal Charges, in order to determine suitability for licensure, the Board has become certified by Department of Criminal Justice Information Services (DCJIS) for access to criminal offender record information for all candidates for initial licensure and license renewal as a social worker. The Criminal Offender Record Information (CORI) Acknowledgement Form  must be notarized, signed with corresponding dates, and mailed to ASWB.

Exam:

Bachelor's candidates must pass the Association of Social Work Boards (ASWB) baccalaureate exam. The ASWB Bachelor’s Exam can be completed with the authorization from the Massachusetts board. Authorization is given following completion of all other application requirements. If an applicant has completed the Bachelor’s exam in another jurisdiction, they should request that verification of a passing score be sent to this Board from ASWB. Scores older than two years will not be accepted.

Licensed Social Worker:

Order official transcripts of your undergraduate coursework once your degree has been conferred. Transcripts must be directly to mass.sw.app@aswb.org.

• Three references are required:
  • Two professional references
  • One field placement supervisory reference to document the placement supervision that was completed as part of the CSWE-accredited BSW program curriculum

At least one reference must be licensed at the LICSW or LCSW level (or equivalent social work license in another jurisdiction). All references must be qualified to evaluate the applicant’s social work experience. Please provide a current email address for each reference so that a secure link can be sent to the references listed on your application.

• A 2” x 2” clear, color photo must be uploaded via the online application, or emailed as an attachment to mass.sw.app@aswb.org.

Clearance:

Upon review of a candidate’s application, the Board will mail an Application Confirmation Letter containing instructions to complete the required Criminal History Check.

Exam:

Bachelor's candidates must pass the Association of Social Work Boards (ASWB) baccalaureate exam prior to applying to upgrade their license from Limited Licensed Bachelor Social Worker to a Licensed Bachelor Social Worker.

Each applicant seeking to become licensed must meet and/or complete the requirements for the Limited Licensed Bachelor Social Worker (L-LBSW) as defined by the State of Michigan. The L-LBSW will be issued for 1 year and may be renewed for not more than 6 times. Additional Training:

Human Trafficking:
Prior to licensing, individuals seeking licensure must complete training in identifying victims of human trafficking that meet Michigan’s standards. More information is available through Michigan's Human Trafficking Commission.

Implicit Bias:
Individuals seeking licensure must have completed at least two (2) hours of implicit bias training within the five (5) years immediately preceding issuance of the license. Implicit bias training must meet Michigan’s standards. For further information review the FAQs.

Post-Graduation Supervision Requirement:

Limited Licensed Bachelor Social Workers must accumulate 4,000 hours of work experience supervised by a Licensed Master’s Social Worker, accrued over no less than 2 years prior to licensure. The qualifying experience must be accumulated in no less than 16 hours per week, no more than 40 hours per week and no more than 2,080 hours in a calendar year. The supervision must include at least four hours of supervisory review of active work functions and records, at least two hours of fact-to-face individual supervision per month and any group supervision included individual review of a candidate’s active work functions and records.

Once complete, the hours and supervision must be verified by the supervisor(s) on the Verification of Social Work Experience for Bachelor’s Social Worker License form.

Clearances:

Candidates must complete a Criminal Background Check. The background check is a comprehensive, nationwide, fingerprint-based criminal background check (CBC). The purpose of a state and national background check is to ensure public safety in an increasingly mobile workforce. Additionally, a candidate for licensure must not have engaged in conduct that was or would be in violation of the social work standards of practice in MN.

Exam:

Bachelor's candidates must pass the Association of Social Work Boards (ASWB) baccalaureate exam. The ASWB Bachelor’s Exam can be completed with the authorization from the Minnesota Board of Social Work. Authorization is given once an application has been reviewed and approved. Students can apply for licensure six months before graduation to gain exam authorization while they complete their program.

Missouri residents are required to show evidence of completing a two-hour suicide prevention course taken within two years of applying for licensure. As a social work candidate in Montana, you must complete the following requirements to apply for the Licensed Baccalaureate Social Worker (LBSW) after graduating from the University of Phoenix Bachelor of Science in Social Work program.

Clearances:

Candidates must complete a background check through submission of fingerprints to the Department of Justice, Montana Criminal Records prior to being licensed. Results of the background check are sent directly to the department by the Montana Department of Justice. 

Exam:

Bachelor's candidates must pass the Association of Social Work Boards (ASWB) baccalaureate exam. The ASWB Bachelor’s Exam can be completed with the authorization from the Montana board. Authorization is given following completion of all other application requirements. If an applicant has completed the Bachelor’s exam in another jurisdiction, they should request that verification of a passing score be sent to this Board from ASWB. Candidates must receive a passing score on the approved ASWB exam within 12 months after receiving written examination authorization from the Board. An applicant must pass the exam within four years of the date of application.

As a social work candidate in Nevada, you must complete the following requirements to apply for the Licensed Social Worker (LSW) after graduating from the University of Phoenix Bachelor of Science in Social Work program.

Clearances:

Candidates must download and complete a Fingerprint Waiver before beginning the application process. The linked waiver is specific to the Nevada Board of Social Work Examiners and provides all required information to the fingerprint official. Fingerprints will be submitted through the LiveScan process. Background checks older than six (6) months old will not be accepted. Candidates will need to scan and email the completed waiver document to the board at slowerv@besw.nv.gov.

Exam:

Bachelor's candidates must pass the Association of Social Work Boards (ASWB) baccalaureate exam. The ASWB Bachelor’s Exam can be completed with the authorization from the Nevada board during the last semester of the social work program. To be eligible, candidates must show proof of meeting the Boards age, citizenship, character, and education requirements. The exam must be passed within 6 months of authorization. If the first attempt results in a non-passing score, the exam can be retaken once, 90 days after the failed examination.

As part of the application process, a criminal background check will be completed and findings could result in the Board denying a candidate. Applicants with prior criminal history should be prepared to submit with their application, a certified copy of court records related to any and all criminal history; and a cover letter explaining the history and the impact of any described events on the social worker both personally and professionally.

All new applicants for licensure are required to submit to a statewide and nationwide criminal history check as well as consent to a child abuse information index check which includes authorization for the ND Department of Health and Human Services to release to the Board reports of decisions that services are required for child abuse or neglect filed. Once the NDBSWE receives the required documents for the criminal history record check and the child abuse and neglect background inquire, they will be submitted for processing. Licenses cannot be issued until the background check results have been received by the NDBSWE. If either of the background checks identify findings or convictions, the license process may be delayed.

As a social work candidate in Ohio, you must complete the following requirements to apply for the Licensed Social Worker (LSW) after graduating from the University of Phoenix Bachelor of Science in Social Work program.

Clearances:

Candidates must complete the BCI & FBI Background checks as part of the application process. Please note, per Ohio law, the Board must deny an application if the Board is informed that the applicant is in default under a child support order or has failed to comply with a subpoena or warrant in a child support enforcement matter.

Exam:

Bachelor's candidates must pass the Association of Social Work Boards exam (ASWB) baccalaureate exam. The ASWB Bachelor’s Exam can be completed with the authorization from the Ohio Board. Authorization is given following receipt of the following:

Follow the eLicense Site process to be eligible to take the exam. Be prepared to provide copies of the following items:

• The completed LSW license application
• Payment of the application and processing fees
• A copy of your driver’s license
• A letter of good standing from your school if you are in your last term of a social work program or
• A copy of your degree or unofficial transcript if you have graduated.

In the event that an applicant does not pass the exam on the first attempt, the applicant can register to re-take the exam in accordance with ASWB polices as long as the applicant’s approval from the Board is valid. Applicants are not required to apply for an exam re-approval if the exam is failed. The initial exam approval is valid for twelve months.

If an applicant has completed the Bachelor’s exam in another jurisdiction, they should simply request that verification of a passing score be sent to this Board, either from ASWB, or from the state Board which originally authorized the exam.

Clearances:

Candidates must submit to a national criminal history record check and it must be sent to the Board at the time of application. Applicants are required to submit two classifiable sets of fingerprints to be provided to the Oklahoma State Bureau of Investigation. If the results of the national criminal history record check reveal that the applicant has been convicted of, or pled guilty or nolo contendere to, any felony, or to any misdemeanor involving moral turpitude, the individual's application for licensure may be disapproved and application declined.

Exam:

Bachelor's candidates must pass the Association of Social Work Boards (ASWB) baccalaureate exam. The ASWB Bachelor’s Exam can be completed with the authorization from the Oklahoma board. Authorization is given following completion of all other application requirements. If an applicant has completed the Bachelor’s exam in another jurisdiction, they should request that verification of a passing score be sent to this Board from ASWB. Upon approval by the Board to take the examination and issuance of a provisional license, the applicant may apply through the ASWB to sit for the examination. From the date of issuance of the provisional license, the applicant has one year to obtain a passing score on the ASWB exam. In the event of failure to pass the examination, the applicant may retake the examination every 90 days during the year the provisional license is valid. If an applicant’s provisional license is revoked, the license application will be voided, and the applicant must reapply, including approval of the Board, application, and applicable fees, prior to retaking the examination.

Clearances:

Candidates must participate in a nationwide fingerprint-based criminal records check. The application is not complete until the Board receives the background check results. This process may take at least 3-5 weeks for processing. Visit Oregon’s LiveScan network for more information.

Exam:

Bachelor's candidates must pass the Association of Social Work Boards (ASWB) baccalaureate exam. The ASWB Bachelor’s Exam can be completed with the authorization from the Oregon board. Authorization is given following completion of all other application requirements. If an applicant has completed the Bachelor’s exam in another jurisdiction, they should request that verification of a passing score be sent to this Board from ASWB. Candidates must receive a passing score on the approved ASWB exam within 12 months after receiving written examination authorization from the Board, but there is opportunity for extension if needed.

Candidates must also take and pass the Oregon Rules and Laws Exam, OR’s jurisprudence exam prior to a license being issued, which is an open book self-test. Applicants will upload the completed exam to the portal application. Per the Board, applicants may take it as many times as necessary to obtain at least a 90% passing score which is a requirement for licensure.

Clearances:

Candidates must complete the Council’s fingerprinting process. Upon submission of an online application candidates will be sent a confirmation email. This email will include a link to the information on getting fingerprinted. The fingerprinting is done electronically at locations all over the country. There is a fee paid to the fingerprint facility.

Candidates with a criminal history must also complete an Application for Criminal History Evaluation Letter to be cleared for licensure. For additional information, please also visit Information for Prospective Applicants with Criminal Backgrounds.

Exam:

Bachelor's candidates must pass the Association of Social Work Boards (ASWB) baccalaureate exam. The Texas Behavioral Health Executive Council has partnered with ASWB to allow prospective licensees to apply for the exam directly with ASWB. Candidates are eligible to register if they have graduated from their social work program or are in their last semester of the social work program. Please note, failure to pass an examination within three attempts will result in an automatic denial of an application. A candidate whose application is denied due to examination failure may reapply for licensure but will not be allowed or approved to sit for the exam again until the applicant has submitted a detailed study plan designed to address the known or suspected areas of deficiency. The study plan must be approved by the Board before authorization will be given to retake the examination.

LBSW candidates also must take and pass a Jurisprudence Examination. The exam results cannot be more than six (6) months old when a candidate applies for licensure.

Exam:

Bachelor's candidates must also pass the Association of Social Work Boards (ASWB) baccalaureate exam. As a social work candidate in Wyoming, you must complete the following requirements to apply for the Certified Social Worker (CSW) after graduating from the University of Phoenix Bachelor of Science in Social Work program.

Clearances:

Candidates must submit two sets of fingerprints for a criminal history background check through the Wyoming Mental Health Professions Licensing Board. The Board will issue the cards and instructions once requested by the candidate.

Exam:

Bachelor's candidates must pass the Association of Social Work Boards (ASWB) baccalaureate exam. The ASWB Bachelor’s Exam can be completed with the authorization from the Wyoming board. Authorization is given following completion of all other application requirements. If an applicant has completed the Bachelor’s exam in another jurisdiction, they should request that verification of a passing score be sent to this Board from ASWB. Candidates must receive a passing score on the approved ASWB exam within 12 months after receiving written examination authorization from the Board. An applicant will only receive three attempts to pass the exam.
